When we think of high-risk in the home setting, we often first think about the drug or therapy being administered such as PN or pain management. But as we know, the complexity of patients seen in the home and alternate site is growing each day. It is imperative to identify all risks that can place a patient in this category, from the therapy, to their co-morbibdities, to psychosocial and mental illness, the list is endless. This presentation will take a look at how to assess patients for risk and review patient case studies, including how one home infusion organization is addressing these patients to provide equitable access to care for all patients.
