  • CLINICAL CONNECT: The Inpatient Nutrition Support Team: An Ally for Successful PN Discharge to the

    This presentation will emphasize verbal communication between home infusion and inpatient nutrition teams to collaboratively review the PN regimen during discharge planning for safe PN patient transition from hospital to home.  Tips for establishing a communication process and key areas of discussion specific to the PN regimen will be reviewed.   Promoting a safe PN patient discharge including use of a standardized PN order template and familiarity with refeeding syndrome risk will also be highlighted.

    Learning Objectives:

    1. Identify specific areas of the inpatient PN regimen that have the potential to be overlooked during transition process.
    2. Understand why verbal communication between home infusion and inpatient teams is vital, and discuss how to establish the communication process. 
    3. Know where to access and how to implement standardized PN order templates.
    4. Identify patients at risk of refeeding syndrome and list preventative measures.
